Derbyshire Carers Partnership Working
We know that Carers want to experience joined up support, to help both them and the person they care for. We also know that in order for people to continue in their caring role they need to look after their own Health and wellbeing and this can mean a wide variety of support.
In order to do this Derbyshire Carers need to work with and to partner with a wide range of organisations, this includes both other voluntary organisations as well as the statutory services such as hospitals, community services and GP practices.
By doing this it improves the information and advice we can give people as well as making more people aware of the potential help available to carers.
Currently we are developing a special relationship with the Alzheimer’s Society in Derbyshire who currently provide the Dementia Support Service. This service is for individuals who have Dementia and their Carers , what we are doing is making sure that we join up with Dementia Support to prevent duplication as well as providing the best information and advice for Carers.
We work with a wide range of other voluntary sector organisations both to help identify carers as well as increasing the help and information we can offer.
We already work closely with adult Social care by having a linked Social worker based in our team this means we can have a far more joined response for carers. We are now working with a range of health related services this includes General practice to help them to become more Carer friendly. We already have some links with acute hospitals such as Derby and Burton Royal Hospitals and Chesterfield Royal Hospital as well as Derbyshire Health Care FT. So this means we link to carers within these services, along with providing information and training to staff to make them better at communicating with Carers. An example of a specific piece of work in this area is how we are working with Derbyshire Community Health Services supporting the delivery of staff training to improve the communication and awareness of Carer issues
