These include; information, advice and guidance, Carers assessments, emergency planning, support groups and much more. If you are concerned about a Carer caring for a family member, loved one or friend living in Derbyshire or feel they would benefit from being involved in their local Carers service, please contact us.

Partnership Working - Work with us

We know that Carers want to experience joined up support, to help both them and the person they care for. In order to do this Derbyshire Carers have created a network of partnerships with a wide range of organisations.

This includes both other voluntary organisations as well as the statutory services such as hospitals, community services and GP practices.

By doing this it improves the information and advice we can give people as well as making more people aware of the potential help available to Carers. If you feel we could work together to improve the overall experience for carers please contact emma.handley@derbyshirecarers.co.uk  

Some examples of partnerships we have established are-

Derbyshire County Council – Adult Social Care

DCA works together with DCC Adult Social Care to ensure Carers in Derbyshire receive the best possible service. From 2017 we have been commissioned by DCC to deliver their Carers Assessment and Support Service.

We complete Carers Assessments, Support Plans and Emergency Plans on their behalf. A DCC Carer Liaison Worker is part of our Carer Support Team and connects both services together for the Carer and the person they care for. We can also ensure referrals are passed to the most appropriate team.

Together we regularly review best practice, gaps in provision and areas for development to create an appropriate and effective range of services.

Joined Up Care Derbyshire

Many Carers access health services for themselves and the person the care for. DCA has a long standing partnership with DDCCG and together explore ways of making services more accessible, joined up and person centred for Carers.

Since 2017 DCA and Links CVS have worked in partnership to support 6 BME Carer Support Groups.

Health

We pride ourselves on the fantastic joined up working we are currently doing with health. By doing this we are improving the overall experience for Carers and preventing carers from reaching Carer breakdown by identifying them sooner.

Some of the work we are doing with health includes: Carers Clinics within GP Practices and CMHTs, Carers Pledge and health specific training sessions for our Carers; such as Dementia Awareness and Delirium Awareness Training.
